Product Description:
A single-platform care coordination solution that simplifies workflow for care managers, engages healthcare consumers and provides data to help providers improve quality and performance outcomes for their patients.
Problem Statement:
Need an user interface where the oncology provider should be able to access the individual patient and help them to set the care plan goal creation.

1. Research Empathizing with the people we design it for. I have designed this module is for oncology providers. And I have identified that their challenges in the area are to monitor patient's treatment protocol variations.
2. Define: There has been a conclusion based on the Empathize. It has been identified that what are their needs and what are their problems and challenges. Based on the observation from the empathize section and turn it into insights that to helps oncology providers improve clinical and financial performance by monitoring treatment protocol variations while monitoring cost and utilization. These cancer care plans meet the individual needs, preferences, and values of patients, vastly improving the quality of care they receive. Providers have access to critical tools that optimize performance, shared savings, quality reporting, and the ability to measure and manage quality metrics for value-based contracts.
3. Ideate: Tried to match the solution with the insights that we figured out with Empathize and Define section on the design thinking process. There had been a lot of new ideas in the white board session, brainstorming workshop and voting process has been held during the exercise (Contribution from Business people, Product Managers and Developers).
4. Prototype: Having the ideas which had been finalized in the Ideate section a low fidelity wireframe is created by sketching in the paper then using Balsamiq rapid wireframe tool. This includes the user journeys and user flows.
5. Test: The created prototype had been sent to the real people which is oncology providers in which we got the feedback in real time.
Challenging and overcome the solution: Client wanted to have all the 15 plans should flow in a single page. And they wanted that provider to set the care plan by add, edit and view the record in a single page. It has been decided that to bring the section in an accordion flow to avoid the scroll within the page. And also provided an action to add, edit, view at the right top of the each section. The wireframe and design has been constructed in such a way that it has been aligned with the existing product design.
